While on a road trip two friends hook up a trucker radio to their car and use it for prank calls. Using the handle "Candy Cane, " the two friends prank a lonely trucker named Rusty Nail and make him think that he's meeting a hot girl at a hotel. When Rusty Nail arrives at the hotel, instead of finding Candy Cane, he finds a rude man. Unbeknownst to the two friends, Rusty attacks the man and rips off his bottom jaw. Afterwards, Rusty decides to take revenge on the pranksters by stalking and tormenting them.
Four friends break down on a highway and walk until they find a farmhouse. Thinking no one lives in the house, they break in and steal a car to get a ride to the nearest gas station. Rusty Nail returns to his house to find his house trashed and his car stolen. He locates the thieves using the tracking device he put on the car, and decides to play sadistic mind games with the thieves as revenge.
Two drug addicts attract Rusty Nail to a hotel room in attempt to rob him. After incapacitating them, Rusty chains them to his truck, telling them if they are able to hold onto his truck for a mile, he'll give them drugs and let them go. After a mile, Rusty doesn't stop, and the female drug addict goes for the drugs taped to the windshield, pulling them both under the truck.

The police find the remains and think that it was wild animals. Later, a group of friends are on a road trip and come across Rusty Nail. The cocky driver cuts off Rusty Nail and almost makes him crash. Frank Theodore Levine (born May 29, 1957) is an American actor. He is best known for playing the roles of Buffalo Bill in the film The Silce of the Lambs (1991) and Leland Stottlemeyer in the television series Monk (2002–2009).
Levine's other notable roles were in the films Nowhere to Run (1993), Heat (1995), Bullet (1996), The Fast and the Furious (2001), The Manchurian Candidate (2004), Memoirs of a Geisha (2005), American Gangster (2007), Shutter Island (2010), Jurassic World: Fall Kingdom (2018), and The Report (2019).

Who were both doctors and members of Physicians for Social Responsibility. Levine's father was of Russian-Jewish desct and his mother had Welsh and Native American ancestry. He describes himself as a "hillbilly Jew."
He became a fixture in the Chicago theatre sce and joined the Remains Theatre which was co-founded by Gary Cole and William Peters. After his stage experice, Levine began to devote most of his ergy during the 1980s toward finding roles in film and television. One of his most promint roles in the 1980s was playing mob forcer Frank Holman in the NBC drama Crime Story (1986–88).
After his breakout role in The Silce of the Lambs (as primary antagonist Buffalo Bill), there was a period where he was typecast in villainous roles. Levine was able to gradually branch out into other types of roles such as a member of Al Pacino's police unit in Heat and astronaut Alan Shepard in the HBO mini-series From the Earth to the Moon. In the drama Georgia, he played Mare Winningham's husband, one of his most sympathetic roles.

In 2001, Levine performed as Paul Walker's police superior Sergeant Tanner in The Fast and the Furious. His résumé also includes an uncredited role as the voice of the sociopathic trucker "Rusty Nail" in Joy Ride, also starring Walker.
Levine provided the voice of the supervillain Sinestro in Superman: The Animated Series, Static Shock, Justice League, and Justice League Unlimited, all part of the DC Animated Universe.
Levine also appeared as a patriarch whose family takes a turn for the worse in the remake of The Hills Have Eyes (2006).

While traveling from California to Colorado to pick up his childhood frid and crush Vna, Lewis is forced to stop in Salt Lake City after he learns his estranged brother Fuller has be arrested. Lewis bails him out and Fuller tags along for the ride.
Fuller has a CB radio installed in Lewis' car and the two begin listing in on truckers' chatter. Fuller coaxes Lewis into playing a prank on a truck driver nicknamed Rusty Nail, asking him to pretd to be a woman named Candy Cane. They set up a meeting with Rusty Nail in a motel where Lewis and Fuller are spding the night; they tell Rusty Nail that Candy Cane will be in room 17, the room of an irritable and racist businessman with whom Fuller had an unpleasant counter at the front desk. Wh Rusty Nail arrives, the brothers list from the adjoining room; an argumt and sounds of a scuffle are briefly heard.
The next morning, Lewis and Fuller learn that the police found the businessman on the highway, with his lower jaw ripped off. Lewis admits they were involved and Sheriff Ritter accosts them for their role in the incidt, but lets them go. Back on the road, Rusty Nail is heard again on the radio looking for Candy Cane. Lewis reveals the prank to Rusty Nail and he demands an apology, but Fuller insults him instead. Rusty Nail th notes they should get their taillight fixed, indicating he is following them.

They drive to a nearby gas station and unsuccessfully attempt to contact Sheriff Ritter. Seeing a large truck pull into the gas station, they flee from the sce into a dead d, with the truck driver chasing them. The driver turns out to be a kind stranger trying to return Lewis's credit card, which he left behind in panic. The real Rusty Nail th shows up in his truck and, as he slowly crushes Lewis's car against a tree, Fuller hysterically apologizes. Rusty Nail drives away, declaring his actions to be simply a retaliatory joke.
Believing themselves safe, the brothers arrive in Colorado and pick up Vna. They stop at a motel and, as Lewis falls asleep, Rusty Nail calls his room, revealing he has noticed Vna has joined them. They flee from the motel, but see messages from Rusty Nail spray painted on road signs, instructing them to look in the trunk; they find the CB radio Fuller had previously thrown from the window of the car. Rusty Nail th contacts them again via the radio, announcing that he has kidnapped Vna's frid Charlotte, and he directs them to a cornfield where the three get split up. Rusty Nail kidnaps Vna.
Rusty Nail sets up a meeting at another motel in room 17, mirroring the false date with which he was pranked. He sets up a trap that will kill Vna if the room door is oped. Fuller attempts to get in the room by a back window, but is injured by Rusty Nail and gets stuck outside. Lewis attempts to free Fuller as the police arrive to help Vna. Meanwhile, Rusty Nail's truck appears uphill and begins rolling down towards the motel. The brothers free Vna in time and everyone escapes as the truck crashes into the motel. As the police investigate Rusty Nail's truck, they see a dead body in the driver's seat and Charlotte, still alive, in the back.

While Lewis, Fuller and Vna are treated for their injuries on sce, the dead man inside the truck turns out to be the fridly truck driver who returned Lewis's credit card. From the CB in the ambulance, the group hears Rusty Nail's voice, learning that he is alive and free.
On the DVD release, there is a 29-minute-long alternate ding, and four other shorter alternate dings. The main one featured Rusty Nail's shotgun suicide and numerous bodies are found by the police in his trailer.
